The following is the Cyclone Support Wellbeing Self-Assessment. The purpose of this short 10-15 minute confidential self-assessment is to help identify possible health and well-being issues that may be affecting you and your success at ISU.

This self-assessment is a helpful tool to enhance your wellbeing and success at Iowa State University. It is recommended to take this in one sitting. Many of the questions and scales in this assessment are part of well researched screening tools or scales used to assess different aspects of health and well-being.  When filling out this assessment, please answer honestly and accurately.  This self-assessment is voluntary and you can opt out at any time. Your specific answers to questions will remain confidential.

When you complete the self-assessment, you will be provided with some personalized feedback and scores on different areas of your health and well-being to review. 

After you review the information, if you are interested in connecting with a well-being coach to get connected to support, you can fill out some additional information and schedule your appointment. If you are required to meet with a well-being coach as part of a university sanction or requirement, you can fill out the additional information and schedule your appointment at that time.
